You should only complete this onboarding form if we have had a discovery call and you have received your She Shines UK Life Coaching Journal. Completing this onboarding form will help me prepare for our coaching sessions. It is also a reflective tool for you to assess what you would like to work towards in your sessions.

What you share on this onboarding form and/or in a coaching session will be kept confidential, unless you disclose something that suggests someone else could be at risk of harm. However, if you disclose that you yourself could be at risk of harm, I will encourage you to get support and try to inform you of services that will be best suited to meet your needs.
